ADMISSIONS OVERVIEW

If you’re 18 years or older (woman or man) and have the desire for a fresh start, you’re eligible to receive services at D’Amore Healthcare. Admission referrals for inpatient care at D’Amore Healthcare can be made by you, a family member, your physician, a case manager or social worker.

Our admissions process is transparent in order to keep your mind at ease and to give you the guidance for your next steps. When you call, we will conduct a 20-minute assessment to ensure our availability to your needs. Our admissions staff will break down every program option available for you or your loved one after an initial assessment. We will talk with you and your loved one about length of stay, arrival options, and payment options. Next, if you are utilizing insurance, we will verify your benefits, review your accommodation preferences and select the D’amore Healthcare Program best suited for you.

To initiate a referral, contact our Admissions Department at 714.375.1110 or complete form for admission and care. Admission to an inpatient rehab program requires the potential patient to meet certain qualification requirements.

WHAT TO EXPECT

D’Amore Healthcare requires the patient’s previous medical history, and his or her current medical condition, as well as the need for and the ability to participate in our individualized intensive therapy program will determine whether a patient can be accepted.

A D’Amore Healthcare professional will carefully review your records and determine admission based on the medical history and needs, the ability to participate in our intensive therapy, and the individual rehabilitation goals.

INSURANCE BENEFITS 

Through insurance verification, we will quickly recognize your coverage. This will lead to payment options to relieve the stress of the financial burdens of rehab. Insurance policies and benefits vary greatly, so it is recommended that a patient provide D’Amore Healthcare with insurance information prior to arrival. Your insurance carrier or managed care company will determine the suitability of care based on your evaluation plus needed care and may certify a portion of a patient’s stay.

Insurance companies run independent of D’Amore Healthcare providing outcomes based on their own insurance reviews, however, our highly trained admission coordinators will work with your provider to determine your benefit coverage and obtain authorization. This might be especially relevant, D’Amore Healthcare does not accept Medicaid or Medi-Cal. Formal reviews do not guarantee payment by an insurance carrier upon final billing.

PLEASE BRING ITEMS THAT WILL HELP YOU GET COMFORTABLE AND CONSIDER ARRIVING WITH THESE IMPORTANT ITEMS:

5 changes of clothes only including undergarments

Athletic shoes
Flip flops and other comfortable casual shoes
Personal hygiene items such as toothbrush, toothpaste, soap, shampoo, hairdryer, and comb or brush
All current prescriptions
Drivers license and passport or personal identification card
Insurance card

IN ADDITION, PLEASE DO NOT BRING THE FOLLOWING ITEMS:

Perfume
Cologne
Mouthwash
Rubbing alcohol
Hand sanitizer
Drug paraphernalia
Any mood or mind altering substances
Also, no clothing that endorses criminal behavior or use of licit or illicit substances

We will provide the following items:
Bedding, linens, towels

D’Amore Healthcare admits patients 7 days a week. Once you or a loved one is admitted, a D’Amore specialist will assist you with an address for all mail and packages.
